As a Case Manager, you will monitor and manage patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. You will be part of a robust and caring multidisciplinary team caring for our patients from admission through transfer or discharge.
Required Skills:• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.• Must be able to work in a stressful environment and take appropriate action.• Demonstrates proficiency in verbal and written communication skills.• Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external.
Position Requirements:Associate's degree in nursing requiredBachelor of Science in nursing preferredCurrent State of MI RN license required1 year of case management experience preferredBasic Life Support (BLS) certification requiredAnnual influenza vaccination or declination required
Position Details:.001 FTE (PRN, variable hours per week)8 hour shifts
