JOB DEFINITION :
Responsible for conducting relationship workshops using the evidence-based curricula. Responsible to engage in activities to locate, engage and intake participants. Assist to provide economic stability services to eligible participants. Maintain effective communication with participants and other service providers to coordinate program activities and ensure Individual Development Plan goals are achieved. Flexibility required for potential nights & weekends to accommodate outreach/workshops/intakes etc.EDUCATION REQUIRED:
Bachelor's degree in human services required; Master's degree in human services preferred.EXPERIENCE REQUIRED:
Experience in a mental health related field, coordination of services, employment/education services, completing collateral contacts.Bilingual in English and Spanish preferred.DUTIES AND RESPONSIBILITIES:
Conduct relationship workshops using evidence-based curricula. Provide support to participants around workshop content as needed both in the office and in the community.
Conduct outreach to recruit/engage potential clients & community organizations serving the target population, presentations to community providers and government entities.
Conduct intakes for potential participants in cooperation with Director both in the office and in the community, including data collection nFORM surveys.
Ensure participants receive services/training in accordance with their Individual Development Plan - including coordination of care with other providers, create individualized employment plan (as needed), documentation of progress, goal achievement.
Support Data Manager with CQI tasks related to the program, including: participant data progress tracking, data sharing with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, work with the entire CN Guidance team to apply the Plan-Do-Study-ACT continuous quality improvement approach to address systemic, organizational challenges, complete QI Training during onboarding and participate in QI refresher training as needed.
Complete program documentation of participants' progress in both client record and electronic health records, including nFORM.
Participate in team meetings as scheduled and partner meetings as needed.
Identify and implement quality improvement trainings and activities as part of professional development goals
Encourage quality improvement processes and provide consistent resources for QI and professional development
