We are looking full time Nurse Practitioner or Physician Assistant to join our experienced and established family medicine practice located in Sylvania, Ohio (minutes from Toledo).

ProMedica Health and Wellness Center was designed to improve our patients' health and well-being by bringing essential medical services under one roof. The three-story, 230,000-square-foot facility houses primary care and specialty physician offices, radiology and laboratory services, a pharmacy, a walk-in clinic, and community room for educational programs. Providing these services from one location not only makes it more convenient for our patients but improves the coordination of the various stages of care.
Responsibilities:
Provide comprehensive primary medical care to patients of all ages, including physical examinations, diagnosing, and treating acute and chronic illnesses, and prescribing medications.Develop and implement individualized treatment plans based on patient history, symptoms, and medical records.Collaborate with a multidisciplinary team of healthcare professionals to ensure coordinated and quality patient care.Educate patients about preventative care, healthy lifestyle choices, and disease management.Maintain accurate and up-to date patient records using electronic health records systems.Stay informed about the latest medical advancements, research, and the best primary care.Participate in ongoing professional development activities, including networking, councils, and boards.

Certificate of Authority to practice is required.Current CPR certification required.Demonstrated excellent verbal and written communication skills.Demonstrated excellent organizational skills.Demonstrated ability to manage and prioritize multiple priorities in a dynamic environment.Must demonstrate and promote a strong commitment toward achieving customer/patient satisfaction.
Physician Assistant:
Master's degreeMust be licensed or license eligible in the state of OhioPA-C
Nurse Practitioner:
Master of Nursing (MSN) degreeMust be licensed or license eligible in the state of Ohio
