Pay Range: $75,000 (depending on experience)Employment Type: Full-time, exempt position with benefitsEmployer 403(b) retirement match plus additional employer contribution (subject to eligibility)Cafeteria plan with medical, dental, vision, FSA, Employer-paid life, and disability insurance16 holidays (10 federal and up to 6 Jewish holidays), annually3 weeks of vacation and 2 weeks of sick leave, annuallyESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

Facilitates initial evaluation and 6-month assessments and quarterly reports for clients attending the centerFormulates the social service component of the Individual Plan of Care and Treatment Authorization Requests for assigned participants in compliance with ADHC standards for establishing medical necessity and providing appropriate careEnsures that social service care plans are coordinated with nursing, physical therapy, and occupational therapy care plans, and fully address the social service needs of clients within the scope of work of a licensed Adult Day Health CareProvides case management to participants as recommended in their treatment plans, or as neededDischarges participants if they are inappropriate for ADHC services; assists caregiver with discharge planPerforms other documentation duties including charting in progress notesThe Social Worker will work the hours necessary to meet the needs of the participants based upon their plans of care and to meet the requirements specified in Title 22, California Code of Regulations, Sections 54329 &78339Provides appropriate information and referral for community members and for all ADHC participants when neededProvides referral for persons not appropriate for adult day health care programReports suspected verbal, physical, fiduciary, and/or sexual abuse to appropriate monitoring agenciesMaintains case records, performs case follow-ups, and closings, and performs other administrative tasks as assigned in compliance with Agency standards, including JFCS’s Policy and Procedures ManualJOB QUALIFICATIONS:

Russian speaking is requiredMaster’s degree from an accredited school of social work; license-eligible, preferredDemonstrated expertise (minimum of two years experience) in clinical casework with the elderly Russian-speaking Émigré populationKnowledgeable about the physical, emotional, and social needs of older adultsProven experience assessing the social service needs of frail elderly and developing care plans to address needs in a manner that enables measurement of outcomesProven ability to document care plans, interventions, and outcomes in an effective manner that meets professional standards for documentation of social servicesProven ability to provide effective case management services to frail eldersExperience providing services in an Adult Day Health Care or comparable environmentExcellent oral and written communication skills in Russian and English requiredAbility to communicate appropriately and effectively with participants, professional staff, families, and caregiversAbility to effectively utilize Microsoft Office Suite software including OutlookPhysically capable of performing the required duties and of assisting frail older adults and disabled adults, as necessaryTo be considered for this position, you must meet the requirements and competencies listed above.
