Summary:Performs specific patient care procedures (such as basic wound care and catheter maintenance) and tasks under the direction of the primary registered nurse or other licensed clinician. All procedures, duties, and tasks are to be completed under the patients care plan as ordered in conjunction with the physician. Patient care primary setting is in the patient home which can include single family residence, Assisted Living Facilities (ALF), or Nursing Home. Patient level of care can range from skilled Home Healthcare, Palliative, and/or Hospice.

Qualifications:

Other information:*High School diploma or equivalent.*1 year of direct patient care in any clinical setting, or in-home health/hospice is preferred.*CNA (Certified Nursing Assistant) certification*BLS*Microsoft Office*Electronic devices, laptop, desktop, smartphone, program applications*Electronic Medical Record on various platforms*Ability to learn and document in EPIC charting system*Good driving record, as defined by Presbyterian, access to a vehicle, drivers license, auto insurance
