**Must-Have:**- Ability to work approximately 1-2 weekend days per month in exchange for a day off during the week- Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)- Minimum of 2 supervisory references covering 1 year of employment total within the last three years within a comparable specialty/setting
**Job Description:**- Participate in the development of the treatment plan- Protect the services process- Organize and participate in family meetings- Arrange durable medical equipment, skilled nursing facilities, and ambulance transport independently- Experience working with health management organizations- Understand admission criteria (InterQual), drug/alcohol abuse treatment options- Manage cases for patients experiencing homelessness or with mental health history- Knowledgeable about advance directives, POLST, adoption, and foster care
**Additional Information:**- Orientation: 40 hours, non-billable- Complete modules pre-start and annually, with self-reported time spent, recorded in transcripts- All travelers are expected to float between all facilities within a given state; refusal to float may result in termination of assignment and/or forfeiture of hours
