Description
We are currently looking for a full time (40 hours, Evening) Clinical Pharmacist at Newton-Wellesley Hospital The ideal candidate will be responsible, trained, and competent for the interpretation, verification, order entry, compounding, and dispensing of pharmaceuticals and pharmaceutical supplies for adult, geriatric, and pediatric patients. The Clinical Pharmacist is also responsible for assuring the safe and effective use of medications, provision of drug information, and patient counseling. They will be responsible for assisting in the development and coordination of all the activities of pharmacy services.The main responsibilities for this role focus on the following key areas:Interprets and evaluates medication orders prior to entry/verificationClinically assesses orders for dose, route, frequency, and appropriateness of medication therapyMonitors orders for drug interactions, adverse reactions, incompatibilities, therapeutic duplication, allergiesIdentifies, mitigates, prevents, and documents problems with medication orders or therapeutic regimens, including medication errors or adverse drug reactionsVerifies the accuracy of order entry and of medication products dispensedDelivers optimal pharmaceutical care by participating in direct patient-care interdisciplinary roundsAdvises nurses, doctors, and patients on the proper and safe use of medicationsNarcotic control and assists in record maintenanceOversees pharmacy technician activitiesAssists in developing and implementing departmental clinical and educational programsAssists in inventory control and proper medication storageCompounds extemporaneous medicationsPrepares IV admixtures as needed. Inspects completed I.V. solutions for accuracy and compatibilityComplies with USP 797 and USP 795 when compounding or overseeing compounded medicationsPrepares prescriptions in accordance with pharmacy laws and practiceMaintains proper licensure and educational requirements and provides documentation upon request
Qualifications
Qualifications - ExternalThe ideal candidate will have a Bachelor of Science Degree in Pharmacy or Doctorate of Pharmacy Degree from an Accredited College/University of Pharmacy.Massachusetts Board of Registration in Pharmacy Licensure is required.1 - 4 years clinical pharmacy experience in acute care hospital preferredCompletion of ASHP accredited Pharmacy Residency preferredBoard Certification (e.g. BCPS Board Certified Pharmacotherapy Specialist or other) preferredKnowledge of medication management of adult, and older adult patients required
