Yours included.The Critical Care, Anesthesia, Perioperative, Extension (CAPE) and Home Ventilation Program is a multidisciplinary team of clinicians, respiratory therapists, and social workers committed to improving the quality of life for children who require long-term ventilator assistance at home. The program creates personalized care plans for children with complex medical needs and coordinates care with primary care providers, surgical specialists, home care nurses, medical equipment companies, and community services.
This APRN will be responsible for :Providing comprehensive and logitudinal service for children with chronic respiratory insufficiency, technology dependence, and related medical complexities.Working closely with each family's primary care provider(s) to create a patient centered medical home, tailored to individual needs.Providing care for infants, children, young adults and families, managing chronic respiratory failure and related complex, special health care needs in the community setting.Working in partnership with the collaborating physician and multidisciplinary team to assess the health status of individuals and families by performing phone triage, teleheath video and in home consultations.Participating with outcomes research to assess program effects and impact of chronic, critical illness on children and families.To Qualify, you must have:2+ yrs experience in the care of children with complex medical needs including tracheostomies, ventilators, and non-invasive ventilation - preferred.May consider other highly motivated candidates with past experience in a PICU or pulmonary rehab center.MA APRN license.PNP or FNP.Ability to work 10 hours/week.Boston Children's Hospital requires all employees to be vaccinated against COVID-19 and Flu, (unless you are eligible for a medical or religious exemption).Boston Children's Hospital is an Equal Opportunity / Affirmative Action Employer.
