Description Providence St. Joseph Health is calling a Inpatient Care Manager to work onsite at our St. Luke's Rehabilitation Institute in Spokane, WA. The Internal Case Manager for Inpatient Rehabilitation, in collaboration with the multi-disciplinary team, coordinates the patient stay and discharge planning from the time of admission to the time of discharge. The Internal Case Manager supports the delivery of quality care in the most cost effective way by managing the human, material and financial resources available for the patient's care needed for individual healthcare needs. The Internal Case Manager is also responsible for ensuring patient and patient's caregiver(s) education in preparation for discharge readiness. Coursework/Training: Sufficient training in psychosocial aspects of physical disabilities and illness. 3 years Clinical experience. Sufficient experience in psychosocial aspects of physical disabilities and illness. Preferred qualifications: upon hire: CCM certification Experience in case management; clinical social work or utilization management. Experience in the rehabilitation of chronically or catastrophically ill or injured individuals. Why Join Providence?
