Nurse Practitioner - Primary Care Outpatient Setting - Allentown, Pennsylvania

- Board Certified Nurse Practitioner with experience- Make an impact on seeing the underserved population- Full-time permanent Primary Care Outpatient Only position- Monday " Friday No nights, no weekends- Small patient panels for consistency and focus-Value-based care model-Provide personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest- Administer Annual wellness visits and health risk assessments, which require aholistic view of health and afocus on thoughtful, accurate, and specific documentation.- Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2years)- Conducting home visits as needed.- This role reports to the Center Medical Director and works closely with operational leadership.-
