Provides case management for athletes requiring medical care. Utilizes clinical expertise, communication and problem solving skills to achieve optimal clinical and resource outcomes. Facilitating referrals and providing linkages to health, wellness, and post acute care resources across the health care continuum. Promotes interdisciplinary collaboration and teamwork to progress the plan of care and discharge plan.
Coordinates care and services within the athletic patient population. Performs patient assessment upon initiation of referral process to identify individualized needs. Assists in identifying aspects of care for the patient/family. Works with treating physician staff to plan strategies to meet care needs. Coordinates plan of care and services, directing liaison activities to appropriately integrate the patient into the health care continuum. Monitors plans of care/pathways/practice guidelines to ensure that expected patient outcomes are achieved within appropriate time frames and utilizing effective resources. Documentation in the medical record is completed in the appropriate time frame and accurately reflects the plan of care and CM interventions planned or completed. Advocates for the patient and family throughout the entire episode of care. Facilitates/coordinates a timely and appropriate discharge plan. Facilitates an interdisciplinary approach to patient care. Participates in Interdisciplinary Team Meetings on assigned units. Maintains effective communications with all disciplines. Provides feedback to the health care team verbally and via chart entries regarding the patient's progress toward reaching expected outcomes or about barriers to the plan. Coordinates changes to the plan as necessary. Assists patients/families with benefits/resources management. Follows patient through Precertification, Financial Counseling and Billing Communicates with patients/families to ensure understanding of third-party payer guidelines and financial implications of care plans. Participates in departmental and system performance improvementInitiatives. Collects and analyzes relevant patient care and fiscal data. Analyzes and evaluates the effect of case management on quality outcomes and fiscal parameters. Complies with all departmental policies and practices and fosters teamwork and professionalism. What We Require: Education:
Registered Nurse; BSN preferred.
Experience:
Five years of recent experience in a clinical health care setting with responsibilities reflecting direct management of patient care including planning, coordination, and delivery of needed services. Prior case management experience preferred.
Licensure, certification, and/or registration:
Current licensure in Virginia as a Registered Nurse.
