As the SNF-ist, you will act as a Skilled Nursing Facility's leading provider of internal medicine servicesYou will collaborate with the facility's interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility's residentsYou will work with the entire facility's patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility's nursing staff and guidance from the facility's medical directors and attending physiciansEvaluation of new-admission visitsCoordination of discharge visitsMonthly stability visits for long term residentsEvaluations for patients exhibiting changes in conditionAs a SNF-ist, you will provide types of care, including but not limited to:Advanced Care PlanningProvide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and diseasePrescribe all necessary medications and treatmentsPerform comprehensive physical examinations of patientsDevelop and implement patient management policies and proceduresFacilitate referrals to other healthcare professionals and medical facilitiesCommunicate with attending physicians and/or specialists regarding patient careQualifications
Advanced Practice Licensure (APN) with a Florida licenseMust have an active license and DEA number.Previous experience in nursing or other medical fields.Ability to build rapport with patients and their loved ones.Ability to thrive in a fast-paced environment.Excellent written and verbal communication skills.Strong leadership experience with emphasis on teamwork in a healthcare setting.Master's degree required
