Visiting Nurse Association of Southeast Missouri is looking for a Physical Therapist to join the VNA Home Health team.

ABOUT VNA HOME HEALTH: The Home Health program provides services for those needing skilled services under orders from a private physician delivered in the patient's private home. Visiting Nurse Association of Southeast Missouri is a non-profit organization.

TYPE OF WORK INVOLVED:

Provides skilled occupational therapy services to those patients/clients referred for such services, and follows the physician's plan of treatment in planning and administering therapy. Assists in problem identification and progression toward specific occupational therapy procedure goals, as well as functional activities based on the patient's evaluation. Communicates changes in patient's condition which warrants a revision in the plan of care to the TCM & Case Manager. Consults with other team members. Provides education to staff as well as to patients./families. Provides/orders equipment (with physician's orders) as required to increase the patient's function and independence and discharge planning evaluation.

TRAINING: VNA provides a 30-day preceptor/orientation training for new hires. This will allow an opportunity for you to shadow our Home Health Team Members for hands-on training. It will also allow time for our Team Members to ride with you, allowing you to ask questions while making patient visits. Computer training will cover the use of the handheld used to make patient visits.

POSITION REQUIREMENTS:

Education:: Degree earned from program approved by the American Occupational Therapy Association.

Experience: At least one year experience as a licensed occupational therapist in a health care institution preferred.
