Sigma Health care is looking for experienced Physical Therapist to work for our contracted facility in Hot Springs.

Shift :

7:30 a.m. - 4:30 p.m.

Role Overview:

The Physical Therapist (PT) supports the treatment of illness and/or injury through the use of physical exercises and therapies, which assist in the rehabilitation and restoration of, body functions to promote maximum client wellness and independence. The incumbent provides therapy programs for patients of all ages with developmental deficits, physical injury or illness. The role may include other responsibilities and duties as assigned based upon the facility needs and requirements.Responsibilities:

Coordinates PT services, providing assigned clients with direct therapeutic care, and teaching rehabilitation skills to the client and/or caregiver that promote maximum client independent functionality and safety. Provides preventive and rehabilitative PT procedures individualized to the client and according to age specific needs. Performs all duties within the scope of the physical therapy practice guidelines and in accordance with productivity standards.Develops and revises a plan of care based on the initial assessment, continual reassessment of client/family needs, client history, diagnosis, physician orders, expected outcomes, and client goals to include discharge plans when appropriate. Performs or monitors the application of treatments involving the use of physical agents such as heat, water, and electricity, using equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound. Follows utilization guidelines and protocols. Formulates and implements appropriate teaching plans, communicates significant clinical findings and changes to the physician and other involved personnel, including clients.Documents all skilled and medically necessary care thoroughly, timely and accurately. Plans of care, documentation, and care provided must comply with Medicare, State, Federal, JCAHO, insurance and payer requirements as well as local policy. Maintains client and organizational confidentiality at all times. Participates in the Quality Management (Quality Improvement) program and completes all required inservice programs to ensure standards are met and skills are kept current.Conducts PTA and Home Health Aide supervisory visits (as applicable), prepares and updates Home Health Aide's written instructions (as applicable) according to local policy. As applicable, supervises Physical Therapy Assistants and Home Health Aides in accordance with Federal/State laws, rules, regulations, and agency policy.Participates in staff meetings, case conferences, business development meetings, and customer service programs as required to ensure the facility meets its goals and obligations to the community. Delivers a high level of customer service to all internal and external customers.Demonstrates leadership competencies through communication and relationship building, developing self and others, and teamwork.Requirements:Minimum QualificationsEducation/Experience:

A graduate of a physical therapy program that is approved by the Council on Medical Education in collaboration with the America Physical Therapy Association.Two years experience preferred.Current BLS.
