What You'll Do:
As an Admissions Coordinator & Hospital Liaison, you will be one of the first points of contact for individuals and families considering admission to our facility. Your role involves guiding them through the admission process, addressing their concerns, and ensuring a smooth transition into our community. Additionally, this individual will travel to local health care establishments to talk with prospective residents for the Pines at Heartwood's assisted living center!
Key Responsibilities:Act as a liaison between prospective residents, families, and the facilityIdentify and develop referral sourcesDevelop and maintain positive relationships with area hospitals and community agenciesGenerate public relations and marketing opportunitiesCoordinate and facilitate the admissions process, including paperwork and assessmentsConduct pre-admission screenings and assessmentsExplain facility services, policies, and procedures to prospective residents and familiesCollaborate with healthcare teams to ensure proper care planning and coordinationMaintain accurate records and documentation related to admissionsHelp individuals and families transition to our care community in an environment where your expertise and dedication are valued and appreciated. Qualifications of an Admissions Coordinator include:
Valid driver's licenseReliable vehicleBusiness Administration degree and/or 3-5 years related experience in long-term careKnowledge of state and federal regulations governing the admission process within a nursing facilityEmpathy for the needs of individuals and families during the transition processExcellent presentation, communication and interpersonal skillsInterest in the nursing needs of the aged and the chronically ill with the ability to work with bothAbility to maintain confidentiality and professionalism in a healthcare settingAbility to work effectively in a team environment
