Job Description:
- Join an in-house therapy team focused on providing high-quality health care.- Work with smaller daily caseloads to optimize time spent with patients.- Handle high-acuity cases responsibly and receive program and certification support.- Develop and implement physical therapy care plans for both long-term residents and short-term patients.- Supervise Physical Therapy Assistants in delivering care plans as needed.- Conduct patient screenings to assess the need for physical therapy.- Communicate progress and any barriers toward therapy goals with the interdisciplinary team.- Ensure timely completion of PT documentation in compliance with regulatory, licensing, and payor requirements.- Maintain rehab gym equipment and treatment areas safely, employing good infection prevention methods.- Provide and document training for patients, families, and caregivers regarding specific care plans and follow-up recommendations.- Specialize in geriatric care within long-term care, nursing home, and rehabilitation center environments.
