The Registered Nurse - Hospice is responsible for providingcare, comfort and support to terminally ill patients. The Registered Nurse - Hospice assumes theresponsibility and accountability for the delivery of hospice patient care invarious settings. The Registered Nursedemonstrates the ability to make clinical judgments in an effective andefficient manner with supervision. TheRegistered Nurse will also demonstrate critical thinking and performanceability in the coordination of patient care. Consistently performs according to nursing standards and is accountablein managing patient care and assisting others in the management of patientcare.
Essential Duties and Responsibilities:
Utilizes the nursing process to assess, plan, implement andevaluate patient carePerforms focused interview to identify specific patientneedsAssesses signs and symptoms indicating physiologic andpsychosocial changes in patient's conditionCollects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnosesDocuments the patient's plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventionsIdentifies and prioritizes actions based upon patient carerequirementsPerforms interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standardProvides a safe, comfortable and therapeutic patientenvironmentInitiates and evaluates patient and/or family education andprovides guidance and supportRevises plan of care according to evaluation, changes inmedical plan of care, effective/ineffective nursing interventionsDocuments the patient's response to nursing and otherinterventionsUses clinical judgment in evaluation activities to meetpatient care needs of an assigned floor/unit including establishing prioritiesMonitors safety and effectiveness of the environment andequipmentAdvises grieving family members on coping techniques andcommunity resourcesPerforms other duties as assigned/necessary
Minimum Requirements:
Current Registered Nurse license for the state in which thenurse practices.Complies with all relevant professional standards ofpracticeParticipation and completion of Amergis' Competency programwhen applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is requiredComputer proficiency requiredMust be at least 18 years of age
