Evaluation -
Obtains pertinent medical and social history. Performs thorough and accurate evaluations using techniques according to current professional and health system standards to objectively measure physical and functional status. Determines functional and measurable short and long term goals and designs an appropriate treatment program. Considers discharge options at time of initial evaluationTreatment -
Performs treatment as outlined in the initial evaluation. Modifies treatment as necessary, updates goals and re-evaluates discharge plans throughout the course of treatment according to current professional and health system standards. Demonstrates thorough knowledge of treatment techniques and applies appropriately. Treatment should include patient and family education and communication/interaction with appropriate team members.Productivity - Productivity requirements and goals will be set forth by the department each fiscal year.Documentation -
Clinical documentation is timely, objective and complete. Administrative documentation including but not limited to billing and outcome measures is accurate and timely.Communication-
Interacts with members of the inter-disciplinary staff pertaining to the welfare and management of assigned patients in a professional manner. Communicates with patients and family in respectful, professional & compassionate manner.Education -
Assumes personal responsibility for continuing education and professional development. Provides education to therapy staff, system employees and community.Maintains up to date with clinical research findings and best clinical practices to practice evidence-based care. Completes required continuous training and education, including department specific requirements.Supervision -
Supervises the patient care activities of assistants, aides and volunteers. Delegates routine care to supportive personnel as appropriate. May supervise students following a minimum of 2 years of employment/experience.
Qualifications
Graduate of an accredited curriculum with a minimum of a Doctorate of Physical Therapy. (For individuals who completed appropriate Physical Therapy education prior to 2015, with a valid Physical Therapy license in the state of practice or proof of license eligibility, a Bachelor's degree or greater in Physical Therapy will be accepted in lieu of the Doctorate of Physical Therapy degree.)Must be licensed by the Connecticut Department of Public Health.New graduate may be considered.BLS certification required
