Medical, Dental, Vision, Retirement Planning (403b), Employee Assistance Program (EAP), etc. Summary With the support and in partnership with the E6 team, the E6 Case Manager will support the E6 team to successfully engage with people currently living on the streets, for the purpose of getting them matched to permanent, affordable housing. Case Managers will walk the streets of Metro LA (Service Area 4) in teams to identify, engage, and assist in housing homeless residents. These teams are part of a new large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). This is a LA County approved strategy to combat homelessness. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. Responsibilities: As part of the weekend outreach team, the Case Manager will be responsible for conducting outreach on Saturday & Sunday as well as three days during the normal work week. Conducting outreach to people experiencing homelessness living in Metro LA (Service Area 4). Assessing the housing needs of people living on the street. Assisting clients in the process of getting affordable housing. Advocating on behalf of client’s needs. Documenting and accounting for time spent on client contacts and activities in client records in accordance with agency and funder standards. Attending team meetings and program-wide meetings daily. Providing support to nurses and social workers on the team with their client needs. Accompanying clients to appointments. Providing resource and referral information to clients. Maintaining appropriate boundaries at all times. Qualifications: An eagerness to provide top-notch quality services to individuals experiencing homelessness. Lived experience with homelessness. An understanding of and familiarity with homeless services and affordable housing in LA county. Experience in the fields of social work, public health, or other types of outreach. Training in harm reduction, “housing first” principles, and street-based outreach preferred. High School diploma or GED preferred. Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record preferred. Physical Requirements: Combination of field and office environment. Regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds. On occasion drive to different sites throughout the assigned outreach area. Will necessitate working in busy and loud environments. Will be exposed to elements like cold, heat, dust, noise, odor, and smoke. May need to bend, stoop, twist, and sit throughout the day.
