The Social Services Benefits Specialist will provide dedicated and specialized assistance for patients experiencing at-risk homelessness that have serious mental and medical issues in supporting SOAR (SSI/SSDI Outreach, Access and Recovery) under the Enhanced Care Management (ECM) as part of the Department of Health Care Service (DHCS) broader initiative of the California Advancing and Innovating Medi-Cal (Cal AIM).Responsibilities:Provide dedicated and specialized assistance for patients experiencing at-risk homelessness that have serious mental and medical issues, and/or co-occurring substance use disorder in applying for Social Security disability benefits.Client base consists of referrals from FHCSD’s Enhanced Care Management and Community Support Team Members.Provide assistance with client base in gathering information to complete SSI/SSDI applications.Gather medical records and other needed information and documentation in completing SSI/SSDI applications.Complete and write SOAR Medical Summary Reports for each individual’s application.Accompany individuals to set appointments at the Social Security Administration as appropriate through each element of the required process.Provide coordination related to visits to medical doctors, psychiatrists, and other healthcare specialists in obtaining evidence in support of the SSI/SSDI application process.Perform other duties as assigned.Requirements:A Bachelor’s degree in a related field preferred.Evidence of ongoing training and education in related areas such as mental illness, substance abuse, and/or homelessness.Ability and means to travel as needed in a timely manner within San Diego County.Must meet driving requirements.Rewards:Job type: Regular Full Time.Competitive Salary with Excellent Benefits.Retirement Plan with Employer Match.Paid Time Off, Extended Sick Leave and Paid Holidays.Medical/Dental/Vision/FSA/Life Insurance.Employee Discounts and Wellness Programs.Sponsoring Employer with an Equity Investment Company that can assist you with your down payment for purchasing a home.FHCSD was founded by community activists working towards a common goal: caring, affordable, high-quality medical care for all. If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors, we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth. We also offer a generous compensation and benefits package.About Us
