Occupational Therapist, Home Health, Full Time, Davis/Weber County, UT

Alta Health Services is excited to announce a full time Occupational Therapist position. This is a hybrid role based out of our Clearfield, UT SNF but servicing patients into Weber County. Our Occupational Therapist will help our patients with the following:Gather background data, review referrals, select and modify evaluation tools, and evaluate patients determining the level of our patient's physical and psychosocial functioning.Develop and revise the plan of care in consultation with the physician and other care-team members.Responsible for planning, evaluation, and treatment implementation of assigned caseload.Preparing clinical notes.Evaluating outcomes.Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.Candidate wil perform Oasis discharges

We are looking for a full time hybrid Occupational Therapist for our home health care/SNF divisions. We offer endless career potential, endless growth possibilities, and competitive compensation.

Our Occupational Therapist will enjoy a competitive hourly rate of $43-$47 as well as a per visit rate ranging between $60-$68. Our OT will receive an extra $10 per hour for drive time.
