1. How long is the contract assignment? 13 weeks 2. Schedule of assignment hours? M - F, 8am - 5pm? Yes 3. Will this candidate be visiting patients in care facilities or only in patient's homes? Possibly will require some care facilities. 4. Will you want to hire the contractor at the end of the contract or is this truly a contract assignment? Contract assignment with the possibility of hiring (CL will direct on pricing) 6. What region are you in? Home health or Hospice? Home Health, Marion County 7. What's creating the request? - work load 8. To which location will this person report for assignment, full address, Including the OU (Location Code) NW 43rd Street Suite C, Gainesville, FL 32606 Please confirm above location is correct and list any other territories that are to be covered by the candidates: Will be covering the county of Marion County for care center above. 9. How many miles per week will the consultant travel? 30/40 a day? Weekly? . What are the weekly units of productivity expectations? 25/30? 30 units 11.
