License: Current license to practice nursing in the state of employment Certifications: Basic Life Support (BLS, American Heart Association), Current automobile insurance, and valid driver's license Must-Have: - Knowledge of terminally ill patients and their families- Understanding of hospice care- Excellent patient assessment skills- Proficiency in managed care principles- Effective operation of nursing assessment tools and common office equipment Job Description: - Provide direct care services to hospice patients and support to their families- Manage an interdisciplinary group of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff- Assure the highest quality of care is provided and documented according to the Plan of Care- Perform initial and ongoing patient assessments- Develop and implement the Hospice Plan of Care alongside the interdisciplinary team and physicians- Work with the interdisciplinary team to offer comprehensive physical, psychosocial, educational, emotional, and spiritual services- Report any changes in patient condition to both the attending and hospice physicians- Maintain accurate and updated medical records- Practice infection control and standard precautions- Monitor staff scheduling- Participate in the Quality Assessment and Performance Improvement Program- Participate in on-call schedules as needed- Make supervisory visits with team members monthly- Ensure compliance with policies, procedures, and regulatory requirements- Educate patients, families, or caregivers based on the Plan of Care- Supervise aides and perform home visits biweekly to assess care and services providedThis position requires a commitment to offering compassionate care and upholding the core values of the organization.
