This position offers an exciting opportunity to participate in the development of a new First Episode Psychosis Outreach Program. This program is part of the Community Mental Health Service of St. Michael's Hospital; a teaching hospital in downtown Toronto. The successful candidate will be part of an interdisciplinary team (psychiatrist, occupational therapist, social worker, registered nurse) that provides outreach, early comprehensive treatment and psychosocial interventions to individuals between the ages of 16 to 24, who are experiencing a first episode of psychosis. The team will implement best practices in relation to assisting family members and other natural supports to help individuals maintain a connection with their community. Research, evaluation and student and public education will be emphasized in the program.
DUTIES & RESPONSIBILITIES:
Provision of Occupational Therapy to promote, maintain or restore health and wellbeing through engagement in meaningful occupations in the areas of self-care, productivity and leisure. Occupational therapy services will include assessment, treatment, consultation, discharge planning and education to patients, family members/caregivers, external agencies and SMH staff.Utilizes assertive outreach strategies, to locate, engage and maintain clients in a helping relationship. Delivers service and supports in the client's environment of choice in the community (e.g. home or other community setting). Adjusts frequency and length of outreach visits based on client needs and goals and team consultationsAble to work effectively as a member of an interprofessional team, collaborating with other disciplines to provide comprehensive patient careContributes to the academic mandate of the organization as an educator, resource or support for staff, students and members of the academic and professional communityIdentifies and addresses educational needs for patients and their familiesDemonstrates commitment to ongoing professional development in order to continue to provide optimal patient careIs committed to, and participates in, ongoing quality improvement in the organizationProvides patient care in accordance with all professional and organizational standardsQUALIFICATIONS:
Must have a Master's degree (or equivalent) in Occupational TherapyMust be currently licensed by and in good standing with the College of Occupational Therapists of Ontario (COTO) for practice in all applicable clinical areas.Current CPR certificationExcellent communication, critical thinking and interpersonal skillsProven flexibility and adaptability in a fast-paced environmentExcellent time management, organizational and prioritization skills as relates to patient careDemonstrated commitment to and involvement with own profession (e.g. membership with professional association, etc.)Knowledge of psychosocial rehabilitation and recovery as it applies to community mental health settingsKnowledge of case management models and case management functionsKnowledge of medical, pharmacological and other treatment strategies for individuals with psychosis and substance abuse disorders
Must be willing to work evenings and/or weekends as directed by the Program.
