If so, you might be the perfect candidate to join our team as a Public Health Nurse at New Hope Clinic in Yakima, WA!

New Hope is a nurse-managed program that is part of Yakima Valley Farm Workers Clinic. New Hope specializes in caring for HIV patients in Central Washington, offering medical, dental, nutrition, counseling, outreach, and mental health services. New Hope also trains healthcare providers on caring for HIV patients and is funded by a Ryan White Part C grant.

We've transformed into a leading community health center. With 40 clinics across Washington and Oregon, we offer a wide range of services such as medical, dental, pharmacy, orthodontia, nutritional counseling, autism screening, and behavioral health. Our holistic model also assists with shelter, energy, weatherization, home visits, and mobile medical/dental clinics.

What We Offer

$38.53-$47.20/hour DOE with the ability to go higher for highly experienced candidatesUp to $10,000 in relocation assistance for candidates relocating more than 50 miles$5,000 sign-on bonus & $2,000 bonus at 12 months of employment100% employer-paid health insurance including medical, dental, vision, Rx, 24/7 telemedicineProfit sharing; 403(b) retirement plan availableGenerous PTO, 8 paid holidays, and much more!As a Public Health Nurse, you will have the opportunity to:

Provide direct nursing care and support to patients and medical providers, including wellness activities like immunizations and screenings.Collaborate with patients and healthcare providers to develop care plans based on medical conditions, goals, and behaviors. Coordinate referrals as directed.Assess and educate patients on readiness for Antiretroviral Therapy (ART) and manage those on HAART therapy.Administer medication and explain its purpose, side effects, and onset of action.Conduct home visits for assessment, follow-up, education, and medication monitoring.Continuously assess, intervene, educate, and refer patients as per the care plan.Maintain regular contact with other services and professionals for coordinated care.Offer HIV counseling and testing to high-risk individuals.Complete chart notes, update EMR data, and document assessments, care plans, actions, and medications.Educate various audiences on HIV transmission and risk reduction.Follow The Joint Commission, HRSA, and YVFWC requirements.Perform additional assigned duties as needed.

Qualifications:

Education: An Associate's Degree in Nursing. Bachelor's Degree in Nursing preferred.Experience: Experience working with HIV/AIDS preferred. Training and six months' experience providing adult physical exams is preferred.Professional Licenses/Certificates/Registration: WA state RN license. CPR certification within 90 days of employment. BLS Certification is required. ACLS Certification preferred. Valid Driver's License and proof of automobile liability insurance coverage.Knowledge/Skills/Abilities:Bilingual (English/Spanish) preferred; Bilingual differential pay requires level 10 on the language proficiency scaleKnowledge of established professional nursing concepts, nursing procedures, and uses and effects of medicationsEffective verbal, written, and listening communication skills.Ability to prioritize work and handle a variety of tasks with frequent interruptionsAbility to interact with patients, families, and employees of all ages professionallyBasic proficiency in Word, Excel, and EMR.
