MHC Healthcare is seeking a Nurse Practitioner - Family Practice (FNP) to join the Integrated Care Team at the MHC Primary Care Health Center, located in Tucson, AZ.

The Nurse Practitioner - Family Practice (FNP) promotes and maintains patients' health by providing medical services to all age groups to the extent which licensed and/or certified. The FNP uses independent judgment in carrying out daily job responsibilities. The position will be supervised by the Chief Medical Officer. MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 14 sites in Tucson and Pima County. The following qualifications are required:

Family Nurse Practitioner master's degree or Doctor of Nursing Practice, with active license to practice in Arizona required.Fingerprint Clearance Card through the Arizona Department of Public Safety (or ability to obtain upon hire)Master's degree (MSN) from an accredited school of nursing, combined with at least 1 year of recent Primary Care or Family Practice experience.Candidate without the MSN, but whose Nurse Practitioner certification is current and was grandfathered-in prior to the Master's degree becoming a requirement for the credential will be considered if they also have required hands-on experience.The following qualifications are preferred:

Experience in a Federally Qualified Health Center (FQHC) or Primary Health Care environment preferred.Equivalent combination of education and experience may be considered if applicable and must be directly related to the functions and body of knowledge required to successfully perform the job.

Ability to effectively present and document information for patient population.Able to respond to questions from leadership, providers, managers, and members of the healthcare team.Must have sensitivity to low income and ethnic minority community.Ability to quickly assimilate, assess and act on upon new concepts, trends, and initiatives.Exhibit strong customer service skills.Ability to represent the organization in a professional manner in a variety of settings, meet people with diplomacy and possess excellent written and verbal communication skills.Ability to make decisions in line with state and federal regulations.Ability to read, comprehend, and analyze documents, regulations, and policies.Ability to prepare and submit complete and succinct documentation necessary to the job.Duties and Responsibilities:

Under the general supervision of the Chief Medical Officer, delivers comprehensive patient care in a primary care setting.Follows evidence based guidelines to diagnose, treat, and monitor acute and chronic conditions.Identifies short-term and long-range patient care issues to be addressed, providing pertinent information, and recommends options and course of action.Refers patients to physicians and specialty providers, as needed, with appropriate documentation necessary to continue care.Collaborates with other providers in the care of patients as needed.Provides continuity of care by developing and implementing treatment plans.Diagnoses and treats common acute health issues.Diagnoses, treats, and monitors chronic health issues.Provides health maintenance care such as physical examinations for all ages, well-baby checks and gynecological care.Collaborates with physician as needed on complex, unusual, and high-risk cases.Orders and interprets diagnostic tests and prescribes proper medications.Follows up with patients through telephone, electronic, and mail correspondence.Documents patient contracts in patient chart.Promotes wellness and self-care of patients and their families by providing health education including, but not limited to counseling on exercise, nutrition, well-baby care, family planning, and diabetic issues.Performs outpatient procedures such as suture removal and other procedures within scope of practice.Attends continuing medical education (CME) conferences/training as required to maintain licensure.Promotes community awareness of health care issues.Promotes and participates in off-site activities such as health fairs, community, and school-based programs as requested by Chief Medical Officer.Ensures proper documentation in patient charting, patient encounter forms, insurance forms and all other additional information pertaining to patient care.Participates in other assigned duties as designated by the Chief Medical Officer and/or Chief Executive Officer.Other duties/projects as assignedBenefits
