POSITION SUMMARY:

This full-time employee manages the operations of the communicable disease team and coverage of the family health team in collaboration with contracted clinical support for program. The employee will focus on ensuring excellence of service, including policy and procedure review and development, quality improvement activities, outcome measurement, and managing daily operations. Supervision, coaching, and developing of staff is a key function of this position. Work is performed under the direction of the director of family health and communicable disease.

QUALIFICATIONS:Minimum of a bachelor's degree from an approved college/university in nursing, and 2 years full-time equivalent experience in a community health, public health, or health care setting, which included management responsibilities; OR equivalent combination of education and experience.

License to practice nursing in Wisconsin.

Driver's license and a driving record that meets County standards; OR ability to obtain reliable transportation.

NORMAL JOB DUTIES:

Leads employees using a results-focused performance management system that provides a framework to encourage employee contributions and includes goal setting, coaching, feedback, and professional development planning.

Contributes decisive leadership and decision-making to the management, communicable disease, and family health teams.

Surveillance of reportable communicable diseases and assures appropriate follow up by team.

Provides consultation to businesses, long term care facilities, daycares, and other congregate settings to prevent disease transmission in an outbreak.

Collaborates with health care providers, including hospital infection control, regarding communicable disease investigations and assuring appropriate treatment.

Implements program plans of work in collaboration with team and community partners.

Carries out outcome measurement activities including, but not limited to, researching best practices, creating and running reports, and monitoring program outcomes.

Reports on grant deliverables under the direction of the health officer or director of family health and communicable disease.

Develops and reviews policies, procedures for programs and services. Assists staff in reviewing, researching, and updating program policies and procedures.

Coordinates and oversees quality improvement activities.

Communicates and coordinates with director of family health and communicable disease, health officer, and other department leadership to assure program coverage and staff support, including providing coverage and oversight to family health team as needed.

Provides team and department coverage on evenings, weekends and holidays as needed for public health events and other program related activities.

Participates in public health preparedness activities and assumes assigned role within the department's Incident Command Structure.

Participation in emergency response to communicable disease or the county's public health emergency response including, but not limited to, pandemic response, outbreak response, and other activities as defined and outlined in the county's Public Health Preparedness Plan.

Participates in management on-call.

Response to a public health emergency or urgent public health matter and on-call response may occur during nights, weekends, and during time not normally designated as office hours. While infrequent, participation is mandatory.

Performs related work as required and as licensed.

EXAMPLES OF WORK PERFORMED:

Oversees the daily operations of the communicable disease team and as needed, the family health team, including staff coaching and development.

Conducts disease surveillance and assigns case investigations; provides coaching and education on diseases and disease investigation.

Coordinates team response to outbreaks; responds to urgent public health matters.

Coordinates outreach programs (immunization clinics, STI clinics).
