a skilled nursing facility in the Eden East Network, is now hiring a Full-time Physical Therapist that can provide rehabilitation services at our facility.
The Physical Therapist is responsible for assessing the need for, developing and delivering physical therapy programs to facilitate rehabilitation.
Primary Job Responsibilities:
Develop and implement individualized therapy programs designed to restore, reinforce and enhance performanceIdentify need for and make referral to other disciplines to address the comprehensive needs of the patientInstruct, educate and train patients and caregivers in the skills and functions essential for promoting patient independence and productivity, in order to diminish or correct pathologySupervise Physical Therapist Assistants, provisional licensees, students and support personnel in accordance with state licensure requirements and professional standardsDevelop care plans using a variety of treatment techniquesCreate fitness- and wellness-oriented programs tailored to patients’ specific needsProvide quality, personalized and evidence-based care and proven interventionsMotivate patients during treatment in order to help them function optimallyPromote clients’ healthy lifestyle by improving strength, flexibility, balance, and coordinationEvaluate effects and monitor and communicate progressSkills
