Clinical Care Coordinator (C3)proactively identifies and removes barriers that my impede department or systemwide patient flow by serving as a consultant and educator with medical staffregarding patient status, bed placement, payor requirements and utilization ofresources. The position (Unit C3) willact as the primary liaison between emergency and surgical services, nursing,behavioral health, finance, physicians, utilization review to ensureappropriate bed is assigned, patient care needs are met and treatment plansexecuted at the appropriate levels. Willwork collaboratively with all members of the healthcare team to improve patientthroughput and length of stay (LOS) performance of the organization.JOBQUALIFICATIONS:Bachelor'sDegree in a health related field is required
Three(3) or more years of clinical experience is required
NurseCase Management experience is preferred
RegisteredNurse in the State of Georgia required.
BasicLife Support (BLS) Certification is required.
ForCritical Care and ECC units Advance Cardiac Life Support (ACLS) is required.
CertifiedCase Manager (CCM) or American Case Manager (ACM) Certification is preferred
