LTSS Service Care Manager

Pay Range: $24 - $27/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• SC will have 3 days of visits and 2 days of documentation weekly
• SC will have a caseload and work assigned tasks
• SC will complete H2060 assessment
• SC will fill put required forms for each assessment
• SC will make follow up calls.
• SC will complete documentation within 3 day turnaround time
• SC will stay organized and ensure work they do not fall behind
• SC will maintain a 90% or high in Audit scores.

Key Requirements and Technology Experience:

• Skills-Social worker, LVN or Case manager.
• Social Service/ Social Work/ Care Management / Services Coordination.
• Nursing/ LVN/ Medical.
• Additional info from the HM: “Social Work, LVN, Medical background that would require at least a bachelor’s for social work and a license for LVN is preferred.
• I will review candidate with a strong medical background without the degree.”
• Note-Remote with travel (2 Days work from home, 3 days travel within 30 -40 miles radius in a week).
• Shift Time & Working Days: 8:00 AM to 04:30 PM CST, Mon to Friday.
