Fairview Health Services is seeking a Nurse Practitioner to join our primary care practice at M Health Fairview Clinic – Brooklyn Park and M Health Fairview Clinic - Bass Lake. The role will involve a split Full-Time Equivalent (FTE) between two clinics, with .75 FTE allocated to practicing at our M Health Fairview Clinic in Brooklyn Park, and the remaining .25 FTE at M Health Fairview Clinic in Bass Lake.
Practice Details:
15 Brooklyn Park provider primary care practice with more than 12 specialists onsite.
9 Bass Lake provider primary care practice.
Patient encounters average 18-22 patients per day.
34 patient contact hours per week.
Openings: 1.0 FTE (Full Time) or less - Flexible schedule.
As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
Fully integrated Electronic Medical Record (EPIC).
Income Details:
Initial 2 year salary guarantee with ability to exceed on an innovative compensation program rewarding production, quality and patient experience.

Qualifications
Graduate of an accredited Physician Assistant program.
BC/BE through NCCPA.
Licensed or ability to obtain a PA license with prescriptive authority through the Minnesota Board of Medical Practice.
Ability to obtain and/or maintain DEA certification for the State of Minnesota.
Meet threshold criteria/qualifications for Credentialing and Privileges.
2 years of Physician Assistant experience required.
