To Assist licensed personnel in the delivery of quality patient care.

• Assists in admission process by obtaining vital signs, providing personal care items and room orientation.

• Places patient’s last name and room number on personal items.

• Ability to identify changes in patients condition, abnormal vital and intake and output.

• Utilizes critical thinking skills in prioritization of care.

• Participates in the development and prioritization of individualized care plan. Provides input to registered nurse.

• Collaborates plan of care with health care team.

• Assist registered nurse in discharge planning by communicating patients participation in care.

• Obtains vital signs as directed by physician and plan of care.

• Obtains intake and output as directed by physician and plan of care.

• Notifies RN/LPN of change in patient condition, abnormal vital and I/O in a timely manner.

• Provides safe environment.

• Includes patient caregiver, and disciplines in care. Documents all patient and family teaching on Interdisciplinary Education Record.

• Demonstrates correct use and troubleshooting of equipment.

• Participates in the evaluation of the plan of care. Documents outcomes met or not met.

• Assists in modification of plan based on patient's response, and provides information to RN/LPN.

• Ability to take directions for RN/LPN. Attempts to resolve conflict independently, if unable, seeks the assistance of the Charge Nurse and the Clinical Nurse Manager.

• Provides health care at the best value without compromising patient care. Utilizes Pyxis appropriately, obtains items under specific patient's name and returns unused items.

• Responsible and accountable for all actions.

• Mobilizes patient's as per physician order. Notifies RN if patient refuses to mobilize, and documents on activity record.

• Offers/Provides bath, linen change to patients who require assistance or complete care, and documents on activity record.

• Provides additional clean wash cloth and towel for patient use during the shift.

• Notifies RN if patient refuses bath and/or linen change, and documents on activity record.

• Observes patient's skin condition and notifies RN of presence of reddened areas or an changes in skin condition.

• Accurately documents patient activity , intake and output and vital signs on the Patient Profile.

• Consistently rounds on patients to anticipate needs.

• Provides ice water at least once every shift to bedside for patients who are able to have ice water. Removes additional fluids from patients on fluid restrictions.

• Passes meal trays in a timely manner, providing wet cloth for patients use. Ensures that all patients who are able, are sitting up and are comfortable for the meal. Picks up meal trays in a timely manner.

• Feeds patients who are unable to feed themselves.

• Follows directions form RN and/or Speech Pathologist for the feeding of patient on aspiration precautions.

• Provides assistance to physician during examination of patient as needed.

• Packages and cleans used equipment appropriately and then transports to Sterile Processing.

• Transports patients to procedure as needed.

• Regularly attends monthly staff meetings.

• Remains informed by consistently reading staff meetings minutes.

• Other duties as assigned.

Required Skills

Minimum Education:

• High School Graduate or GED equivalent.

• Completion of Nurse Assistant program preferred.

Required Licenses:

• Certified Nurse Assistant preferred.

• Must have certified from the Kentucky board of nursing.

• BLS Certification - American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.

Required Experience

Minimum Work Experience:

• **** Nursing Assistant experience preferred.

Qualifications:Minimum Education:

• High School Graduate or GED equivalent.

• Completion of Nurse Assistant program preferred.

Required Licenses:

• Certified Nurse Assistant preferred.

• Must have certified from the Kentucky board of nursing.

• BLS Certification - American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.
