Description

The NP functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs). By using the SPs the NP is able to change treatment regimen after observing signs and symptoms of illness, reactions to treatment, general behavior and physical condition and determines if they are abnormal findings (Ca Nursing Practice Act, subsection(b) of Section 2725). The NP will provide and coordinate the care of patients in the ambulatory care setting, skilled nursing facility setting, home setting and assisted living/board and care settings within a geriatrics practice. The NP will provide patient/family education and promote safe continuity of care. The NP will participate in interdisciplinary and clinical rounds, provide education and support, make appropriate referrals and documents services rendered.

The NP will provide and coordinating care for geriatric patients in various settings, including ambulatory care, skilled nursing facilities, homes and assisted living setting. The NP will also assist with managing geriatric-related inbox messages.

Salary range: $84.82-$121.13/hourly

Qualifications

BLS from the American Heart Association or American Red Cross - required

Current, valid CA Nurse Practitioner License - required

State of California NP, State of CA NP Furnishing with Schedule II, National Certification (i.e.ANCC) - required

Master's or Doctoral Degree in Nursing, NPI #, DEA# - required

Enrollment in PECOS, Enrollment in CURES - required

Adult and Geriatrics experience - required

Previous NP experience - required

Ability to establish and maintain cooperative and collaborative working relationships with administrative, clinical personnel - required

Ability to set priorities - required

Document patient care in a concise and comprehensive manner - required

Ability to utilize excel, word and other software programs - required

Skill in working independently and following through with minimum direction - required
