The Transportation Driver is responsible for the overall transportation and safety of patients. They will assist with housekeeping duties when no patients are in need of transportation.

Description of Primary ResponsibilitiesResponsible for the overall transportation and safety of patients to and from the health centers.Communicates with the Health Center and Navigation Center staff to ensure patients are properly transported in the most efficient way.Communicates with other Transportation Drivers staff to ensure patients are picked up to the health center and transported back to their homes in an efficient, and safe manner.Maintains daily transportation logs and submits them weekly.Supports transportation services at other sites, as needed for coverage.Adheres to the Preventive Maintenance Schedule for the transportation vehicles.Coordinates with the automotive repair shop to ensure proper and timely vehicle maintenance.Ensure that transportation vehicles are always fueled and ready for use.Keeps mileage record and repair records up to date.Maintains transportation safety standards and daily cleanliness of transportation vehicles.Completes the daily vehicle inspection list.Reports any traffic violations and accidents.Obeys all traffic violations and respects other drivers and vehicles.As part of other duties, performs housekeeping duties when not transporting patients.Performs other duties as assigned.Description of Primary Attributes

Professional & Technical Knowledge:

Must possess a high school diploma or General Educational Development (GED) certificate and be at least 21 years old.Job duties require specific knowledge of transportation processes and practices, typically learned on the job, or which may include a series of training sessions that would comprise a few weeks if done consecutively.Technical Skills:

Ability to prepare basic correspondence and simple reports in Microsoft Word.Ability to use Microsoft Excel to create tables and simple displays of information.Ability to create basic presentations in Microsoft PowerPoint.Licenses & Certifications:

Must have a valid "Class A or B" Driver's License with Passenger Endorsements if driving a 12 or more passenger vehicle, and a clean DMV record that allows employee to be eligible to be insured to transport patients by FHCN's automobile insurance carrier. In lieu of a "Class A or B" Driver's License, may have a valid "Class C" Driver's License to drive an 8 passenger or less vehicle.Communications Skills:

While performing the duties of this position, the employee is regularly required to stand or walk. Pay Scale:

Min Hourly Rate: $21.00

Max Hourly Rate: $28.60
