Physical Therapist, Skilled Nursing Facility

MedAdventures is looking for a skilled Physical Therapist to join one of our largest Skilled Nursing Facility partners in the Allentown, PA area!
This role can be Part time OR Full Time 40 Hours a week with a very flexible schedule. Any additional help is being considered.
Compensation and Schedule:Target Pay Range $55/HR W2 + $1,000 Assignment Completion BonusComprehensive benefits optionsFlexible Schedule is M-F Day Shift
ResponsibilitiesScreen/Evaluates all residents to determine need for Physical Therapy.Establishes an individualized treatment program with consultation to facility staff in areas of Physical Therapy.Provides results of the evaluation and treatment objectives to caregivers, family members, and residents.Completes all documentation as required by the fiscal intermediary, within the designated time frame.Implements the treatment program based on evaluative results.Monitors the resident's response to treatment and alters the treatment plan as appropriate.Supervises Physical Therapy Assistants and Rehab Aides.
Required Qualifications:Bachelor's Degree or Master's Degree in Physical Therapy from an accredited college or university.Current and valid license granted from state of employment.Previous experience in long-term care preferred, but not required.
Required CredentialsCurrent professional PT licensure in good standingProfessional certificationsTB documentation - Must be 2 step conducted in the past yearHepatitis B vaccination, titers, or declinationAnnual statement of health within previous 12 monthsCriminal background check - PATCHFBI check required if therapist has not lived in PA for more than 2 yearsSkills Checklist - Our internal requirement - client upon requestOIG check - Exclusion from state or federal program screening
