Pet insurance.Paid vacation timeMileage reimbursementEmployee referral bonusesFlexible assignments, autonomy and work-life balance1:1 social worker-to-patient ratios where you impact outcomesAs a Hospice Medical Social Worker, here’s a big-picture view of what you’ll do:Provide home-based social services to patients dealing with end-of-life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition or a lack of resources that make managing their journey difficultWork as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT or SLP, focused on the patient’s plan of care and individual goalsMonitor a patient’s psychosocial condition and identify social and emotional needsConduct patient assessments, document progress and ensure patient is moving toward goalsProvide counseling, community resource planning, crisis intervention and advocacyConsult with family and caregivers on patient’s plan of care and how to help them progressA few must-haves for Hospice Medical Social Workers:Graduate of a master's program in social work accredited by the Council on Social Work EducationLicensed to practice as a Social Worker in the state in which employed.
