Case
Manager: Provides Case management service for both inpatient and observation
patients assigned. Performs a comprehensive assessment of psychosocial,
medical and discharge needs of patients/family, coordinates the plan of care
and drives the discharge plan by collaborating with the multidisciplinary
team. Evaluates progression of care using InterQual throughout the episode of
care.

Social
Worker: Works collaboratively with RN case managers and the multidisciplinary
team, performs assessments and develops appropriate discharge plans for
high-risk patients with discharge planning or psychosocial needs.
Engages in consultation, discharge planning,
education, counseling, advocacy, and information referrals to meet the
emotional, social, and financial aspects of care.

Able to remain calm in crisis, ability to deliver information in a
clear concise manner, ability to build trust and act with authenticity.
Ability to multitask and prioritize work flow and needs. Ability to adapt to
changes, team player.
