JOB SUMMARY:
Provides high quality care to each assigned patient/resident with daily nursing care and services in accordance with the resident/patient’s assessment and plan of care.

Essential Duties & Responsibilities:
Assist or complete resident activities of daily living according to the care plan for assigned residents. Activities of Daily Living include, but are not limited to the following:
o Bathing (tub, shower, bed)
o Dressing
o Toileting (bathroom, bedpan, commode, urinal, foley catheter care, incontinent care)
o Transferring (in and out of bed, chair, wheelchair, lounge chair, tub, scale, etc)
o Transporting (wheelchair, walker, ambulation)
o Lifting
o Eating (tray pass, feeding, recording consumption, supervision/encouragement, nourishment pass)
o Range of Motion exercises
o Grooming (hair, shaving, nail care, oral care)
o Bed making
o AM and HS care
o Provision of privacy
o Restorative Nursing measures
o Insure a safe environment
Safely use equipment (lifts, scales, restraints, side rails, splints, positioning devices, arm protectors, specialized mattresses, adaptive equipment) Obtain and record vital signs(temperature, pulse, respiration) and weight. Maintenance of dignity, individuality, confidentiality and resident’s rights Explain to resident what care you are going to provide prior to providing it. Caring for residents with communicable diseases who require isolation or special precautions. Wearing appropriate personal protective equipment and following infection control procedures. Maintaining resident’s room, bath and personal belongings in a clean, orderly fashion. Assists with admissions and discharges as directed by the nurse. Reports to the team leader at the beginning of the shift, listens to report of the resident’s condition, receives assignment, asks questions and reviews the resident’s care plan. Reports off to team leader when leaving the unit. Adheres to assigned break and meal times. Attends in-services as directed Reports perceived problems to the supervisor Is responsible for any duties, within the scope of practice of a CNA, as requested by the team leader, nursing supervisor, ADON, DON. Follows the facilities policies and procedures Follows Universal Precautions and Infection Control practices Maintains Corporate Compliance standards Contributes to facility Quality Assurance Performance Improvement initiatives
Education Required High School or better Licenses & Certifications Required CNA
