Under the supervision of a registered nurse, the licensedvocational/practical nurse medical surgical assumes responsibility andaccountability for the application of the nursing process and the delivery ofpatient care. Consistently performsaccording to nursing standards and is accountable in managing patient care andassisting others in the management of patient care.
Essential Duties and Responsibilities:
Under the direct supervision of a Registered Nurse: utilizes the nursing process to assess, plan,implement and evaluate patient careIs aware of physiologic and psychosocial changes in thepatient's conditionCollects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnosesDocuments the patient's plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventionsIdentifies and prioritizes actions based upon patient carerequirementsPerforms interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standardProvides a safe, comfortable and therapeutic patientenvironmentDocuments patient's response to nursing and otherinterventionsUses clinical judgment in evaluation activities to meetpatient care needs of and assigned unit/floor including establishing prioritiesMonitors the safety and effectiveness of the environment andequipment • Performs other duties as assigned/necessary
Minimum Requirements:
Current Licensed Practical/Vocational Nurse License in statepracticingComplies with all relevant professional standards ofpracticeParticipation and completion of Amergis' Competency programwhen applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicableCurrent Health certificate (per contract or stateregulation)Must meet all federal, state and local requirementsSuccessful completion of new hire training as applicable tojob siteUnderstand patient confidentiality and HIPAArequirementsAbility to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is requiredComputer proficiency requiredMust be at least 18 years of age
