Primary Location: Winters, CA; additional locations: Esparto, CA (~2 days/week)
Projected Hire Date: January 2025
Hours per week: 24-40 (we are open to discussion)
Commitment bonus potential of up to $82,500 for FT:
* Up to $32,000 internal bonus for 3 years commitment and based on FTE* Up to $50,000 available to individuals outside of Yolo county, Solano County, and PHPs remaining 22 counties.

If you were a Family Practice Provider with Winters Healthcare, you would be one of our six care team providers, but would operate independently amongst a primary care team at our facility in Esparto and Winters. "Team [insert your name here]," includes a compassionate Medical Assistant, a motivating Population Health Coach (PHC), and a designated Patient Service Coordinator (PSC) working side-by-side for your panel of patients. You will work alongside our Behavioral Health Consultants for warm hand-offs and other behavioral health support. To enhance our integrated care teams, and resources for patients, we also have our dental department right across the lobby, as well as an on-site pharmacy and staff pharmacists.
Our services include adult and pediatric primary care, chronic disease management, prenatal care, and minor procedures. Your day will vary from visit to visit: an 18-month-old well-child visit, to a 63-year-old uninsured individual with a cough, to a diabetic migrant-seasonal farm worker, or a 25-year-old coming in with their first pregnancy. Each care team typically sees around 17 - 25 patients in an 8-hour shift, and have patient panels that average around 700. We place emphasis on establishing trusting relationships while being devoted to quality health outcomes and we have the data and measures results to prove it.
The ideal Family Nurse Practitioner has a deep commitment to community health and will possess a blend of family practice clinical expertise, with prenatal care up to 36 weeks as a plus! Responsibilities:
* Communicate, collaborate, and work in a team-oriented environment supporting our relationship-based model of care* Responsible for providing primary healthcare services/assessments to patients and families focusing on healthcare maintenance, disease prevention, early diagnosis, treatment, and patient education and counseling, in an efficient and compassionate manner.* Performs health assessments, CHDP physicals, gynecological procedures, and basic lab testing, including interpretation of results* Adheres to industry standards of practice in the delivery, oversight, management and documentation of services provided* Communicate and collaborate with colleagues to ensure quality patient care and a commitment to professional development
* Documents clinical observations in patient medical records. Provides first aid/emergency care as appropriate. Manages and oversees all medical, pediatric, and obstetrical/gynecological categories for cases that do not require referral.
Qualifications:
* Current FNP license to practice in the state of California, as well as valid DEA, and CPR/ACLS/BLS;* Minimum of 3 years clinical experience, with comfort to be the only medical provider on site (Supervising physician's available for consult)* Bilingual fluency in English/Spanish is preferred (ability to read, write, and speak)
Compensation and Benefits:
* Annual equivalent salary range: $136,700 - $159,000 for FT & based on qualifications and experience* Health insurances: Medical, dental, vision, life, AD&D - 100% covered for employee, and up to $2,400 towards dependents* Paid Time Off (PTO), Floating Holidays, and paid holidays off. Family Nurse Practitioner
Reports to: Medical Department Manager
(Clinical supervision is under the Medical Director)
Summary of Duties
The Family Nurse Practitioner (FNP) works within a model of team-based care and is responsible for full-scope family practice services including prenatal, and family planning services. Under the supervision of the Medical Director, the FNP will perform complete examinations of patients and screen all patients for health maintenance needs, including immunizations on an integrated healthcare team providing relationship-based model of care. The FNP will document clinical observations, adhere to industry standards and conduct chart reviews to ensure compliance.
Essential Functions:
* Responsible for providing primary healthcare services to patients and families focusing on healthcare maintenance, disease prevention, patient education and counseling* Performs health assessments, CHDP physicals, gynecological procedures* Performs basic lab testing, including interpretation of results.* Orders appropriate pharmacological interventions* Actively participates in quality assurance and quality improvement efforts and process improvement programs* Participates in meetings and completes administrative tasks as assigned* Assists in the recording of no-show visits, lab results, and prescription refill requests* Support quality improvement and performance improvement efforts, and participate in related activities as required.* Performs other duties as assigned
Minimum Requirements:
Education and Work Experience
* Current license to practice as a Family Nurse Practitioner in the State of California.* Minimum two years' experience in a public health arena, preferably in a community clinic setting* BCLS, ACLS, or CPR Certification (or receipt of certification within 60 days of hire)
Knowledge, Skills and Abilities:
* Bilingual fluency in English and Spanish is preferred; basic Spanish terminology is required.* Strong organizational skills and attention to detail, with the ability to build and maintain relationships with patients, colleagues, and the public.* Experience with standard office equipment and computer literacy, including proficiency in Microsoft Office (Word, Excel, Outlook).* Understanding of cultural diversity and its impact on healthcare delivery, particularly in underserved communities.* Capacity to handle a diverse range of patient cases and adjust to the dynamic needs of the practice.* Ability to work independently as well as amongst an integrated care team* Effective oral and written communication skills, along with strong interpersonal abilities.* Emotional intelligence and critical thinking skills.* Demonstrated ability to work collaboratively in teams and achieve shared goals.* Proficiency in using data and evidence-based practices to inform clinical decisions and improve patient outcomes.* Proven background in providing exceptional customer service.* A passion for and commitment to meeting the healthcare needs of our clients.
