The Oncology Nurse Navigator (ONN) serves as an expert clinician with advanced training and skill to manage the complex needs of patients and their families through evidence-based practice per NCCN guidelines. The ONN is responsible for managing care of identified patients with cancer, in collaboration with the multidisciplinary team, to ensure access to high quality care throughout all phases of care. The ONN serves as point of contact for cancer patients, caregivers, and physicians to provide resources and assistance with accessing clinical and supportive care services. Serves as a liaison between the different medical specialties to ensure best practice. Responsible for data collection to ensure the facility meets standards for accreditation with COC and NAPBC. The ONN is responsible for utilizing data to identify health care disparities with patient population.

Requirements:Minimum 5 years of oncology, navigation, palliative care, end of life or hospice experience requiredGraduation from an accredited school of nursing and licensed as an RN by the Louisiana State Board of NursingBSN strongly preferredOncology Nursing Certification (OCN) or Oncology Nurse Navigation Certification (ONN-CG) is preferred or completed within 3 years

Responsibilities:Conducts an assessment of patient, family and/or caregiver's needs at pivotal times across the care continuum to identify problems, distress screening results, barriers to care, and/or limitations. Communicates and refers the information to other members of the healthcare team.Assists patient and caregivers to anticipate, identify and overcome barriers within the healthcare system and facilitates referrals as appropriate to mitigate barriers.Collaborates with the medical providers to coordinate patient treatment plans and facilitates timely coordination of care communicating concerns to the appropriate physician or health care provider. Serves as liaison and facilitates communication between patient, family/caregiver and the healthcare team.Educates patient and family/caregiver on cancer diagnosis, treatment, side effects, symptom management, cancer center support services and community services. Provides education and resources to facilitate informed decision making, timely access to quality health and psychosocial care, and improved adherence to treatment.

Full-Time; Days

Monday - Friday

7:30 AM - 4:00 PM and 8:00 AM - 4:40 PM

Pay Range:

Non-Exempt/Hourly

$30.49 - $48.00
