Education:Successful completion of a Nurse Practitioner program; Master's in Palliative Care preferred. Licensures/Certifications:Current registration with the Maryland State Board of Examiners of Nurses as a Nurse Practitioner, specializing in Adult, Family or Geriatrics; Inpatient position can include Acute Care certification.National and State Certification as required for the relevant position (DEA, CDS)Completion of "healthcare provider" CPR courseCertification in Hospice and Palliative Care through Hospice and Palliative Credentialing Center (HPCC), Advanced Certified Hospice and Palliative Nurse (ACHPN), or equivalent, preferred.Center for Advancing Palliative Care (CAPC) Designation, preferred.
Experience:At least five (5) years of related experience in a healthcare environment.At least one (1) year related experience in Hospice and/or Palliative care.Skills:• Skill in using computer, familiarity with the use of electronic medical records• Ability to interact with patients and families who are vulnerable and dealing with life limiting or serious illness issues.• Effective written and oral communication skills to communicate and relate effectively with patients, family, staff in facilities, the public, physicians, and insurers.• Analytical skills in order to assess patients and implement appropriate intervention, care plans, and treatments• Able to work under varying degrees of stress and the ability to adjust to changes in the work environment.• Skill in leading, motivating and developing others• Skill in teaching nurses, physicians and other clinical and non-clinical staffSkill in applying principles and practices of integrative medicine and health.

Principal Duties and Responsibilities:• Assumes responsibility for the evaluation and consultant recommendations for treatment and care plan of patients in assigned areas.o Performs initial consult as evidenced by appropriate documentation.o Writes recommendations in accordance with GBMC/GGL policy.o Suggests orders related to symptom and pain management.o Discusses with the attending physician, nursing and social work staff physical and/or emotional findings, which may interfere with care of the patient.• Evaluates patient progress on a continuing basis and in collaboration with the attending physician, and alters treatment modalities as needed.o Evaluates nursing, medical, social and educational needs on assigned patients.o Initiates referral to appropriate ancillary departments and community resources based on patient need and documents on the progress note.o Writes/Types progress notes for each visit documenting pertinent information regarding patient progress and treatment regime.o Writes orders appropriate to the patient's care requirements in compliance with existing contract.o Establishes medical diagnoses in compliance with contract.o Refers to the physician or designee those problems which require consultation and documents same on progress note.o Makes immediate referral of acute and or life threatening patient conditions to emergency department or hospice as appropriate per patient care plan. Documents physical findings and treatment plan pertaining to same.• Performs technical functions as covered by the job description and attestation statement submitted to BPQA and/or MBON, approved NP attestation and in accordance with GBMC policy.o Documents results of technical functions performed.o Reports to physician or designee any abnormal findings related to it when appropriate and if indicatedo Monitors patient response related to technical function performed and documents it.o Acts as a clinical resource.o Actively supports and participates in departmental performance improvement initiatives• Performs activities to meet patient/family educational needs.o Participates actively in health teaching of patient and family to include preventative health care as well as current medical condition and treatment regime. Evidence of same by documentation on progress note and/or established flow sheet.o Review discharge planning with patients, families, and nursing staff.o Reviews and updates MOLST form and goals of care with patient, and /or POA and physician when indicated and as needed.• Assumes responsibility for utilizing opportunities for educational advancement by pursuing activities to meet identified educational needs and those required for continue certification as well as attending mandatory in-service as required by department and GBMC.• Acts a liaison for the GGL group to our facility partners.
