You will play a vital role in delivering compassionate and effective care to our patients in post-acute care communities. Your responsibilities include:
* Providing comprehensive primary care services to a manageable patient panel of 15-20 patients per day* Collaborating with interdisciplinary teams to optimize patient outcomes* Ensuring smooth transitions of care with hospice, palliative care, and home care agencies* Maintaining accurate and timely documentation in our EMR system (Gehrimed)

Qualifications:
* Graduate degree as a nurse practitioner or physician assistant* Active and unrestricted NP or PA license in North Carolina* Strong clinical assessment and diagnostic skills* Excellent communication and interpersonal abilities* Experience in post-acute care preferred, but not required (training available)
Expected compensation is estimated salary range of $104,000 to $135,000 with no cap productivity income potential.
