Nursing Coordinator- Structural Heart Ambulatory (Full-Time) Days-Detroit

GENERAL SUMMARY:
Reports to the Nursing Leader/Administration. The Nurse Coordinator is a Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients. Coordinates care across the continuum to facilitate quality outcomes. Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction. Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient.
Structural Heart Disease is a new, rapidly growing innovative area of cardiac care. The Center for Structural Heart Disease at Henry Ford is a nationally recognized "Top 10" in the country program. The SHD RN Coordinator is an essential member of the SHD team, coordinating the care of patients with valvular heart disease, through an evaluation process, following them from initial touch to completion of treatment. Beginning with evaluation of new referrals, the SHD RN Coordinator works with providers organizing care, monitoring patient response to treatment, preparing patients and family members for complex catheter based procedures, provides education to patient and family members, continuously contributes to process improvement. We are seeking Registered Nurses, with a minimum of 3 years' experience in cardiovascular care. Ready to be an active professional member of our Structural heart team.
Hours: 8-hour days Monday-Friday (may be 7:30 am- 4 pm or 8am - 4:30 pm)
Location: Detroit
EDUCATION/EXPERIENCE REQUIRED:
Graduate from an accredited school of nursing.
Successful completion of HFHS (Parent Operating Group) Pharmacology test.
Maintains and demonstrates current clinical knowledge in area of specialty.
Bachelor's Degree in Nursing preferred.
A minimum of three to five years of progressively more responsible clinical nursing practice in specialty.
Certification in specialty preferred.
Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
Ability to perform a wide variety of professional nursing services.
Ability to effectively communicate with patients, families, and medical group personnel.
Preferred experience in the Cardiology.
Basic computer knowledge and keyboarding skills.
Knowledge of Word and Excel.
CERTIFICATIONS/LICENSURES REQUIRED:
Current licensure by the Michigan State Board of Nursing as a Registered Nurse.
Current BLS or ACLS per HFHS Policy.
PHYSICAL DEMANDS:
May require heavy lifting of patients and/or equipment.
WORKING CONDITIONS:
Works in a patient care setting with possible exposure to communicable diseases.
