Job Description
Purpose of Position:
Under general supervision and in accordance with New York State requirements for written practice protocols and written agreements or for collaborative relationships with qualified physician(s) or licensed health care facility (see Note section), serves as a licensed, independent practitioner responsible for planning, coordinating, delivering and managing comprehensive health care services in a health care setting. Ensures patient and family-centered quality care to improve outcomes and patient experience, in accordance with applicable NYC Health + Hospitals policies and procedures, state and federal rules and regulations and nursing standards of care.
Responsibilities:Provides continuous comprehensive medical and nursing care, including preventive and supportive health care services to patients and families.Manages health problems and coordinates health care for various patient populations. Identifies and diagnoses illnesses and physical conditions, and implements therapeutic and corrective measures.Consistent with the established nursing practice, differentiates between normal and abnormal findings in the screening process to detect changes in health status that require physician's intervention.Develops and maintains partnerships with community-based organizations and makes referrals to consulting physicians and/or other health care professionals and community agencies, as necessary.Evaluates and interprets patient data, in order to plan and implement appropriate nursing interventions.Consults with physicians and other health care practitioners in the evaluation of health screening findings to make prospective decisions about treatment that will best support improved outcomes and patient satisfaction.
Minimum Qualifications
For Assignment to Nurse Practitioner1. A valid New York State license and current registration to practice as a Registered Professional Nurse; and2. Current certificate as a Nurse Practitioner issued by the New York State Department of Education(NYSED); and3. A current certification issued by the State Education Department to prescribe medication.
For Assignment to Nurse Practitioner (Primary Care/Behavioral Health)1. In addition to meeting the qualification requirements of Nurse Practitioner, must be assigned to aPrimary Care or Psychiatric/Behavioral Health setting
