Maternal Child Health Nurse / Welcome Family / Early Intervention Parenting Partnership - Full Time

Pay $39.00 - 41.00 / hourly

The employee serves as the home-visiting Nurse for the Welcome Family Fall River Program and the Early Intervention Parenting Partnership Program to ensure early identification of maternal/infant risk, provide health and safety education, and linkages to services to prevent or mitigate poor health and/or developmental outcomes.

ESSENTIAL FUNCTIONS:

Program Specific Duties:

Welcome Family

* Schedule and conduct 90 minute visits at the familys home or by telehealth to enrolled families who have recently given birth at Charlton Memorial Hospital, or those who reside in Fall River.* Screen in six core areas: clinical assessment, unmet health needs, maternal and infant nutrition, substance use, emotional health, and domestic violence.* Engage, assess, and present information in an unbiased, culturally appropriate manner, particularly related to sensitive topics.* Assess the familys needs and strengths related to overall health and well-being.* Provide brief intervention and education to all families.* Offer resources and referrals based on familys needs.* Conduct a follow-up contact call with all caregivers 2-3 weeks post initial visit.* Expectation of no less than 9 visits to be completed per month (PER DIEM POSITION)

Early Intervention Parenting Partnership:

* Assess the familys needs and strengths related to overall health and well-being to create an Individualized Family Care Plan.* Make appropriate referrals for additional services based on families needs.* Provide direct home-based services for participants.* Maintain assigned productivity level on a monthly basis.

General Duties:

* Work collaboratively as a member of the Welcome Family and Early Intervention Partnerships Program teams to conduct outreach to community partners and referral sources, coordinate service delivery, and build program capacity.* Complete documentation and billing according to the program and Department of Public Health standards; ensure security and confidentiality of program data and materials.* Collect and enter data demonstrating proficiency with technology, organization, and engagement.* Participate in ongoing supervision.* Complete required DPH training sessions, including in-service training, MA-DPH Learning Collaborative meetings, and annual site visits.* Perform other duties as assigned by the supervisor, division management team member, or division director.* Participate in all staff meetings and required training sessions, including in-service training, as scheduled.* Maintain all program and Agency required training.

QUALIFICATION REQUIREMENTS:

* Current licensure as an RN by the Massachusetts Board of Registration, Division of Professional Licensure; Associate Degree required, bachelors or masters Degree in nursing preferred.* Minimum of 2 years clinical experience in prenatal, newborn, infancy or maternal services with knowledge of: Community-based, public health nursing; Newborn care and womens health issues; Recognizing the spectrum of postpartum mood disorders through assessment and observation; Understanding the dynamics of domestic violence and substance abuse; Providing culturally responsive care; Identifying the risk factors for child abuse and neglect, including the mandatory reporting laws; Infant behavior assessment, including educating parents around infant development; Motivational interviewing techniques; and Trauma-informed care.* Ability to work and communicate effectively within the Agency and the community* Ability to provide a current copy of all required health records( for EI).* A qualifying criminal background check, driving record, a valid drivers license and dependable transportation required.* Bilingual and bicultural candidates encouraged to apply.
