Position Summary:
The Nurse Practitioner will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention.
The Nurse Practitioner must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's individual needs. The Nurse Practitioner will provide a safe and confidential environment for patients, families, and personnel.
Essential Functions:
* Interview patients to obtain accurate, thorough health histories.* Performs physical examinations and provides preventive health services.* Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs.* Establishes diagnostic impression.* Formulates comprehensive health care plan based on findings.* Prescribes or recommends medications or other forms of treatment.* Refers and consults with other professionals as needed.* Educates patients and family members about health care issues.* Ensures appropriate patient follow-up and reevaluation occur.* Actively participates as a member of the integrated health care team, working conjunctively with the School Based Care Clinic, Care Team, and dental and behavioral health departments to ensure seamless, multidisciplinary patient care
Additional Requirements:
* Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.* Complete all required training as designated by HR and the position supervisor.* Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.* Comply with all SCCC Vaccine Mandates.* Willingness to travel and work at other SCCC locations as determined by the supervisor.* Comply with all applicable Credentialing and Privileging requirements timely.* Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.* Complete additional duties as assigned by your supervisor.* Must participate in quality assurance/ improvement activities when applicable.* May be scheduled for Saturday clinics and telehealth hours.

Education/Experience:
Must Haves to be Successful:
* Master's Degree or Doctorate in Nursing.* At least two years experience in a family medicine or community health center setting and DEA preferred,* Interpersonal skills and sound judgment are necessary to communicate with a wide variety of persons, including the indigent, and to work with the Clinic staff in providing the medical services of the Clinic.* Exhibit cultural competence and exceptional teamwork skills.* Ability to read, write, and clearly speak the English language.* Ability to interpret a variety of instructions and deal with multiple linguistic and cultural variables.* Ability to relate cooperatively and constructively with patients, co-workers, administration, physicians and providers, community agencies, referral sources, regulators, and other health team members.* Ability to enlist the cooperation of others.* High tolerance for stress.* Excellent organizational and leadership abilities.* Outstanding communication and people skills.* Knowledge of the industry's legal rules and guidelines.

* Previous non-profit and/or volunteer experience preferred.* Bilingual speakers are highly preferred.
Work Environment:
The work environment is in a healthcare setting and can be fast paced and involves communication with patients and medical personnel. This role is highly clerical in nature.
