POSITION SUMMARY

The Advanced Practice Provider (APP)/ Street Medicine is a Nurse Practitioner (NP) or Physician Assistant (PA) primarily responsible for working collaboratively with other health professionals within the Street Medicine program in providing high quality medical care to our unhoused patients as a part of a team-based model. They will provide exceptional patient care and will be skilled in networking and collaboration. This position will work with contracted Street Medicine vendors, the MLK Community Health Foundation and national and international entities for MLKCH Street Medicine as programmatic needs arise.

ESSENTIAL DUTIES AND RESPONSIBILITIES

* Participates in strategic planning within overall needs in function of growth and clinical enterprise of MLKCH Street Medicine* Facilitates relationship with other practitioners and learners* Develops and educates learners* Provides input for recruitment, onboarding and education of new staff* On-boards new clinical provider hires* Provides input to the Manager of Population Health and Street Medicine for performance appraisals for staff* Provides ongoing education to staff, learners and community on health and homelessness* Participates in development of new protocols for street medicine along with the Manager of Population Health and Street Medicine and medical director* Participates in the development of standards for clinical application of new or established guidelines for patients* Participates in local and state street medicine organizations (national and international optional)* Participates in the development of quality standards for clinical operation in Inpatient and Outpatient setting along with the Manager of Population Health and Street Medicine and medical director* Oversees quality of patient care with input from the clinical program leader and medical director* Ensures accurate tracking and dispensing of pharmaceuticals* Responsible for ensuring timely and accurate documentation* Responsible for determining appropriate scope of practice with input from Manager of Population Health and Street Medicine and Medical Director* Responsible for implementation of quality performance activities along with Manager of Population Health and Street Medicine* Working collaboratively with Manager of Population Health and Street Medicine facilitates team perspective for staff at MLKCH, MLKCHF, and in the community* Assists in development of MLKCH Policies and Procedures with input from Manager of Population Health and Street Medicine and medical director.* Perform other duties as assigned or requested.

POSITION REQUIREMENTS

A. Education

* Graduate from an accredited Nurse practitioner program approved by the California Board of nursing (MSN and/or DNP) or Graduate from an ARC-PA accredited physician assistant program* California State Medical License, DEA* National Provider Identifier (NPI)* If Nurse practitioner, must have current certification via American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). Current and without restriction RN and NP license required.* If Physician assistant, must have current certification by National Commission on Certification of Physician Assistants (NCCPA) and current and without restriction Physician Assistant license.

B. Qualifications/Experience

* Current Basic Life Support (BLS) for Health Care Providers from the American Heart Association* Current Advanced Cardiac Life Support (ACLS) for Health Care Providers from the American Heart Association* Minimum experience is 1-year experience as an APP, or at least 3 years of RN experience, preferably in acute care setting.* 3-5 years of clinical experience as APP in related field (preferred)* Must complete annual Workplace Violence Prevention Program/Certificate, per hospital policy, during initial training/orientation but not to exceed 30 days from hire/transfer.* Valid unrestricted CA Driver's License

C. Special Skills/Knowledge

* Bilingual skills preferred (Spanish)* Basic computer skills
