Visiting Physician Services specializes in treating adult and geriatric homebound patients. We make house calls to private residences, senior housing buildings and assisted living facilities in Ocean, Monmouth, Middlesex, Somerset, Union, Essex, Passaic, and Bergen Counties.Our practice is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion .We are currently looking for a Nurse Practitioner to join our team to cover sections of Ocean County. The position would be responsible for ensuring delivery of high quality care practices in the treatment of symptoms and promoting the highest quality of life for homebound patients and their families. Responsibilities
1. Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers2. Assesses, diagnoses and develops appropriate plan of care to treat homebound patients3. Monitors the patient's condition and responsiveness to the treatment provided4. Acts as an agency representative with patients and their families as well as with other health agencies, groups and organizations5. Participates in organizational wide QAPI program and attends agency sponsored in-service programs6. Adheres to agency infection control and safety policies7. Adheres to the organization's policy in regards to absenteeism and appearance
Qualifications
1. Master's degree in Nursing required from an NLN accredited program2. Licensure as an Advanced Practice Nurse (APN) in NJ with prescriptive authority; maintain a CDS and DEA 3. National certification as an Adult or Geriatric Nurse Practitioner4. 2 + years of APN experience and/or significant experience as a registered nurse5. Excellent communication, organizational and interpersonal skills6. Strong computer skills
Working Conditions/Physical Demand: Working indoors/outdoors with Client/Customer contact and clinical equipment and use of phone and computer; valid driver's license and automobile insurance
