Description:

RN: Completes comprehensivenursing assessment of all residents on admission and on an ongoing basisaccording to state and federal regulations using the nursing process(assessment, diagnosis, intervention and evaluation). Demonstrates competencein clinical skills as identified on Competency Verification and TrainingChecklist. Assures assigned residents receive ordered medication, treatmentsand other procedures within defined time frames, utilizing appropriateinfection control procedures. Demonstrates familiarity with the therapeuticeffects and side effects of all medications administered. Obtains equipment /supplies to provide care as needed. Assures procedures are followed forpressure ulcers, dehydration, malnutrition, incontinence, pain and medicationadministration. Accurately completes glucometer testing and control monitoringaccording to procedure. Implements specific approaches to deal with behavior,enhances residents’ mood state according to resident care plan. Assuresresident care for assigned residents is completed through reviewingdocumentation and updating as necessary. Uses the EMR accurately. Documentsactions and notations for all resident condition changes according to policyand procedure. Documents all actions taken related to admission, discharge,transfers or death. Documents incidents or accidents in resident record and allnecessary forms (incident reports, etc.) Completes behavior documentationaccording to policy and procedure. Completes seven-day observation tool.Assists/participates in quality improvement process, including audits andcorrective procedures as assigned. May complete the assigned portions of theMDS for Nursing and sign the MDS forms to signify completion of the process.Assures physician's orders are processed and physicians notified in a timelymanner. Makes rounds with physicians as assigned. Attends or obtains residentreport prior to beginning shift. Receives and shares information; observes andreports residents’ conditions. Responds promptly to resident call lights.Directs and assists with resident, family/significant other education. Notifiesfamily on condition changes etc. Reports / assures timely follow up of signs /symptoms of resident infections.

LPN: Assists with formulating a plan ofcare for residents in pertinent area. Provides nursing care to residentsaccording to physician orders and regulations. Updates plan of care whenneeded. Observes and documents observations and care given to residents. Usesthe EMR accurately. Assures physicians are notified and orders are processed.Receives and shares information; observes and reports residents’ conditions.Responds promptly to resident call lights. Directs and assists with resident,family/significant other education. Notifies family on condition changes etc.Handles linens properly and disposes of soiled linens, clothing or supplies inadherence to regulations and guidelines. Uses standard precautions by usingpersonal protective equipment and observes transmission guidelines, washeshands between resident cares. Reports / assures timely follow up of signs /symptoms of resident infections

Qualifications:

RN :

Education Experience Licensure Certification:

Graduate of an accredited school of nursing. Prior work experience in like or related position preferred. Maintains required certification / qualifications and associated standards. Licensure for state of practice is required.

Knowledge:

Knowledge of basic nursing skills; knowledge and familiarity with the therapeutic effects and side effects of all medications administered.
